    The Vaccination Certificate of COVID-19

       In order to prove that you have been vaccinated with the vaccine of COVID-19, we issue a Vaccination Certificate of COVID-19 by paper.

       * We issue it from FY2024 onwards,but,we only issue those that can be issued are limited to those with vaccination dates before FY 2023. 

    Who can apply?

       Persons who meet the following requirements;

       1. Persons who have been vaccinated with the vaccine of COVID-19 in Japan.

       2. Persons who were registered as residents in Ami Town on the day of vaccination.

       * If you have registered as a resident in another municipality on the day of vaccination, you need to apply to that municipality.

    How much is the fee?

       No fee

    How to apply?

       The application method is one of the following.

       (1) By visiting the reception desk

       * You can visit the reception desk in the “Sawayaka Center” (Address: Ami 4671-1, Ami town) and apply directly. Reception hours are from 8;30 a.m. to 5;15 p.m. on weekdays.

       * Even if you apply at the reception desk, the delivery will be by mail. Please note that you cannot get The Vaccination Certificate on the same day.

       * If you are in a hurry, please make a reply envelope into Express mail (“Sokutatsu” by Japanese) by attaching express postal stamp of 260 Yen in addition to basic postal stamp (usually 84 Yen).


       (2) By mail

       * Please note that it may take up to 2 weeks from your sending to your receiving due to postal circumstances.

       * If you are in a hurry, please make your envelopes (both send and reply) into Express mail (“Sokutatsu” by Japanese) by attaching express postal stamp of 260 Yen for each envelopes in addition to basic postal stamp (usually 84 Yen).

       * You have to send application documents to;

    What do I need to apply?

    By the person oneself

       1. Application form

       * When you fill this, put check mark on "International travel & domestic use in Japan" on a Type of certificate.

       2. A copy of your passport (page with your passport number)
       * The passport must be valid at the time of travel.

       3. A copy of the vaccination certificate given at the time of vaccination
       * Not required if lost, but delivery may be delayed.

       4. A copy of the identity verification document with the return address stated (e.g. Residence Card, My Number Card, Driver's License, etc.)

       5. Reply envelope attached postage stamp (Please write the return address.)

       6. A copy of the agent's identity verification document (e.g. Residence Card, My Number Card, Driver's License, etc.)

    CAUTION

       1. Person's name and passport number on the vaccination certificate must be same them on the passport, because it is assumed that the vaccination certificate and the passport will be checked by matching them at the destination.

       2. If your passport will expire by the scheduled travel date, first you need to renew your passport and then you need to apply based on your renewed passport.

       In addition, other documents may be required when entering destination or boarding an airplane, so please contact the embassy of your destination country or airline  for details.
